What can I say?  My mother is in her second stay at PNRC and is thriving. We had her in another facility previously, (Tunnel... just check the Yelp comments and draw your own conclusions) and she wound up back at the ER after their "care". Beware!!! Do not let you loved ones go there! Her care at Pacifica is professional, warm, caring and upbeat. The facility is immaculate. I have nothing but raves to say about her Physical Therapist, Greg, Occupational Therapists: Rhonda and Susan, Administrators: Jacob, Beth, Philipina and other staff: Ruby, Arthur,Arlene, Mary, Rosa, Noe, Nary, Florinda. Why do I know all their names? The place is so caring and and the staff, so personal it feels like coming home- but also like a nice hotel. When she arrived there was a gift basket awaiting her!
This last year my mother has been through hell. She has had several horrible falls, a spinal chord bruise and two agonizing surgeries. I felt that this team really fought for her... insurance, doctors, caseworkers. She lives in a really lovely and well-known San Francisco home that she longs to come back to... but was eagerly anticipating coming back to this place until she was well again. My family has always taken care of our own until our elderly pass on. I was ashamed that my mother's injuries were beyond what I could care for. Thank God for this place that has helped her be well and thrive until she can come home.  Thank You Pacifica!  George, Amber, Catherine and all our friends.

Mr. Shawn McAffee, Administrator
Scottsdale Nursing and Rehabilitation
3293 Drinkwater Boulevard
Scottsdale, AZ 85251

Dear Shawn:

I wanted to take a moment to let you know about the positive experience my family has had with Scottsdale Nursing and Rehabilitation. On Sunday, January 23, 2011, my father was discharged from the hospital with very little notice. I was asked to find a Skilled Rehabilitation Facility for him and I had a few hours to do so. I called a few places, and with it being a Sunday, I was unable to get very far. However, when l called Scottsdale Nursing and Rehabilitation, Liz answered the phone on the first ring which was wonderful. I noticed immediately that her attitude was friendly and helpful, yet concerned and professional. She described the facility briefly and I was impressed. She answered my questions in an intelligent and thoughtful manner which gave me a feeling of confidence. She said she thought you could help and she invited me in for a tour.

From the moment I entered the parking lot, I felt good about the facility. The curb appeal is immaculate. The windows are clean and the landscaping is attractive. The parking lot is free of clutter and debris. When I entered, I immediately noticed the bright, pleasant and colorful atmosphere. Jennifer introduced herself and her demeanor was upbeat and positive. Several of the nurses said hello and were friendly. I was excited to see a large, well equipped, rehabilitation center. I noticed a professional and caring demeanor in the staff which I have come to realize is the cornerstone of the facility. All of the staff members were dressed appropriately and were either working at a station or were actively engaged in helping the patients. The hallways are pretty and bright. I was quite pleased. As a matter of course, I did tour two more facilities, but I decided that the best place for my father’s rehabilitation would be your facility.

The admission process was handled quickly and professionally. Liz and Jennifer worked hard in get the paperwork complete. They answered my questions efficiently and the process was seamless. Upon admission to the facility, my father was disoriented and confused. The staff members who got him settled (Debbie and Sean) handled him with the utmost of skill and experience. I left knowing I had made the right decision. My father has now been in the facility for 11 days and his progress has been fantastic. I have visited the facility several times and have met several more staff members who are all pleasant, upbeat, hardworking and professional - just an incredible group of people. Each time I visit my father is actively engaged with other patients or in some type of therapy. He has gained weight, is walking better and is responding well. Your entire staff has displayed an incredible amount of patience, care, and skill. Each and every employee takes his/her job seriously. I’ve noticed that the staff members get along well with each other they work together in a cohesive manner. They are friendly and professional to family members. Above all, I feel that your staff really cares about my father and his progress - something that any family member values above all in their quest for quality care for a loved one. I believe that the leadership of your facility is top notch and that transcends amongst the entire staff and to the patients and families. In summary, my brother and I could not be more pleased to have our father in your care and it is no surprise that your facility has earned top ratings.

It will be my pleasure to highly recommend Scottsdale Nursing and Rehabilitation in the future. Please feel free to contact me at any time or to give my name as a reference to others. Lastly, thank you for providing the care and compassion that have made a difference for my family and our loved one.
Most sincerely,

Nina C.

Park West Care Center
Re: Elynore R.

My mother, Elynore R., lived at Park West for four years before she passed away on February 3rd.

I wish to express my heartfelt ‘thank you” to the second floor staff for their conscientious care of my mother while she was a resident. She was kept comfortable, and she was happy during her time at Park West. The staff always welcomed me and my husband, Bob, and shared information with me about how my mother was doing and how much they liked her.

I want to extend a special “thank you” to Theresa Lopez, and her second floor staff, for their compassion and gentle care during the last few months of my mother’s life. Theresa is a generous and caring person, and you are fortunate to have her on your staff.

Burien Rehabilitation and Nursing Home Mar 7, 2011
Mike Anbesse and Staff

I am writing just to thank you and your staff for caring for my morn during her short stay there while trying to get better. While we have to admit no one wants to be in a nursing home but my mother was starting to come around and the very day she passed she seemed to be getting into the groove of things there. We all realized that she was very sick and was hoping only for the best, we did not expect her to die this soon, it was the farthest thing from our minds especially after we had talked that day about her progress getting better. Mike I want to thank you personally for having the care and passion for your job and the true concern for your staff and patients. Not often enough do we run across the staff that will go the extra mile to do whatever it is and tirelessly to make sure the patients are cared for and taken care of. This person is you Mike, if you could get paid monetary for the job well done, it would not be enough. The talks we had about mom and her care were much appreciated and will be never forgotten. You offered your phone number 24/7 and often asked what else I can do to make your mother comfortable and happy, you made it personable like it were your own mother. Not often enough are we thanked for a job well done and this is why this letter comes to you to make sure you and your staff are properly thanked. This truly made me feel that you really cared. The grief that we share as a family and sadness will fade away in time but never forgotten because to lose your mother is overwhelming and a big loss. Each and every one of us is trying to deal with this loss and I am having a hard time because we were very close. With prayer and God everything will work out and therewith be peace within each and every one of us. Mike keep up the good work and they have the right person to lead the rest of the staff in the direction. You are a great asset to the nursing home, this type of job can be very difficult without the care for the elderly and people as a whole. I will not hesitate not even for one second to refer any family needing care for their loved one to Burien Rehab if that ever presents itself. Take care and keep up the good work

I have great respect for the Administrator, the front office and all workers who are employed at Valley Skilled Nursing Facility.

June 2, 2010

Dear Julie and Staff,

Six and one-half years a go, our family made the best decision of our lives in placing Bill and Dorothy in Courtyard Care Center. They both enjoyed living there together for five years before Bill's passing on December 22, 2008, and Dorothy has continued to call Courtyard her home for the past one and half years, with nary a complaint from either of them in all that time. (Except maybe when Bill was asked to walk while in therapy because of the pain of bone against bone in his knees.)

Under the wonderful direction of your mother, Amelia, and now you, Julie, we have felt only kindness, and consideration from every single staff member. You are undoubtedly the best care center around, and we love you all! We do not hesitate to recommend you to all.

Through the years, our family has enjoyed many of your fabulous celebrations. We thank you for each wonderful luaus. Bill and Dorothy's beautifully planned anniversaries especially their 75th and 78th; the oustanding celebration in 2004 when they were chosen King and Queen; and the many birthday parties. But now, we thank you for the best celebration of all - Dorothy's 100th birthday. It was perfect in every detail and we thank you all for the part you played in making a most special day for our mother and also for friends and family. We will always hold this wonderful memory in our hearts for as long as we live. Thank you all.

Our love,

The S. Family

As crippled as I was, I did not think there was a chance I would ever walk again. My primary therapist, Steve, told me that to walk again I would have to work very hard. I was at Apple Valley for 2 ? months. Steve, the therapy team, and the whole staff cheered me on every step of the way. I left Apple Valley 23 pounds lighter and in much better shape physically. I feel 10 years younger. With a walker, I am walking on 2 legs. Thanks to a great staff. You guys are the very best.

Big thanks to everyone for taking such great care of my dad during his post-op stay.  This was his first experience under full time nursing care, and we were so concerned about how he would respond.  But thanks to Beachside staff always being so warm and respectful, he did very well, and remained there for as long as he needed to be.very sincere thanks again for everything!

After arriving at Brentwood three weeks ago, I didn't know what to expect or how long I was to stay. I had fear of what awaited me. Within days the therapists had begun their rehab work on me. They tracked my progress each day and adjusted my therapy accordingly. All the therapists are skilled, friendly, caring and eager to help your progress. I highly recommend all those I associated with and feel that you have a super group of technicians. Thank you for all your help.

I would like to thank you for the great care my mother, Donna S. received at Chatsworth Park Care Center. She was there for over three months recovering from a fractured knee. She was in station 2 and the staff was wonderful the way they treated her. I was there at the Care Center once a day and sometimes two. I had first had experience of your staff. That includes the front office, nurses, housekeeping, CNAs who were there at my mother's beckoned call she enjoyed the food and the service with the food staff. My mother also received great physical therapy five times a week. From the admission by Jonathan C. to Mary D. arranging home health for her upon her release, EVERYONE was wonderful. Donna is now at home walking independently as she did before her knee fracture.

As you probably heard, my mother was practically in tears when I came to pick her up on Friday, July 27th 3pm because she did NOT want to leave! She loved being at your Center because of the care and the other residents there. So to accommodate my mother, I returned at 8:30pm to pick her up. She left reluctantly. I have told other people this story including the home health provider and they have never heard of someone that wanted to stay at the rehabilitation center!

Ms. Anderson-Paul, thank you for the great rehabilitation my mother received at your Center. You provided great care for my mother and assurance for me that she was being well cared for.

Sincerely,
Barbara S., daughter

”I want to thank the entire staff at Coventry Court for the truly excellent work everyone has done, not only caring for Dad but for being responsible for his remarkable recovery and rehabilitation.  When first arriving at Coventry, Dad’s condition was very serious and the outlook for a significant recovery bleak.  Within two weeks under your care, I noticed a pronounced improvement in my father’s ability to communicate and could see the beginnings of him regaining cognitive functions.  He began to eat better and participate in rehabilitation treatments designed for him.  From that time onward, Dad steadily improved in his physical condition and mental abilities...Thank you all very much for what you’ve done in taking care of my father, and congratulations on doing it so extremely well.

I would like to thank the Fairfield Rehabilitation Nursing Home for providing exceptional medical care and supportive services to my Mother, Ms. Eunice B. The medical and administrative staff members exemplified the true efforts needed to comfort my mother whom suffered from a multitude of medical conditions during her length of stay.

The skills and expertise of each person I came in contact during my mother's stay made me feel well assured that she was in the best care, and confirmed my decision to place her in the care of the Fairfield Rehabilitation Nursing Home was best.

Although my mother preceded her family in death just over a week ago, it would be unfair if the individuals whom provided her medical care during the last months of her life were not recognized. My mother was a fighter until she was called by the Lord to start her work in heaven. Please know if I were ever to recommend someone to a nursing home, the Fairfield Rehabilitation Nursing Home would serve as my number one recommended service provider.

Again, thanks so much for your facility's active participation in the provision of care for my mother. All of your work will forever remain deeply appreciated. The entire family will always consider the Fairfield Rehabilitation Nursing Home a special place.

Very Sincerely,

Dear Ms Mannon,

My grandfather, John P., passed away Sunday in your facility. He had been a patient for about four months.

During his stay at Fairmont, I visited him and his wife (also a patient) numerous times during the week and weekend, both during the day and the evening. I was able to observe how the nursing staff cared for him and other patients. I was very impressed with the professionalism and compassion displayed by the staff.

With specific reference to my grandfather, his caregivers were always sensitive to his needs and encouraged him to be as independent as possible. They showed great respect for his privacy. It was difficult to see this once vibrant man in declining health but each time I left the facility I felt that I was leaving him with good people.

This level of performance also included your business and front office staff. They were easy to work with and very helpful.

One final note- the staff’s concern extended to me and my family. On the night before he died, my parents, my sister, my wife and I were gathered around my grandfather who was by that time comatose and non-responsive. At about 8 p.m. one of the staff came in with a plate of cookies and a pot of coffee for the family and she actually apologized that the kitchen was closed and this was all they could do. We were greatly touched by this kind act and it is an example of what we experienced during my grandfather’s four months.

As a person also involved in the delivery of health care services, I know how hard it is to consistently deliver high quality services. Your staff does that.

Yours truly,
Gary J. P.

As impossible as it seems, we brought mom to Villa seven years ago. This was not a snap decision. Prior to bringing her to you, my wife, Rhonda and I visited numerous facilities in the area. What we saw at Villa was an open, clean, uncluttered and non-offensive smelling facility. Believe me, this was not the case in some of the other places we visited.

The family feels that the care Patti received at Villa was outstanding. Everyone was professional, caring, and loving, not only to Patti, but to the family. The staff at Villa did its very best to accommodate her and make her happy.

The bedside periodic meeting with staff and mom also gave the family a feeling of being involved and comfort in knowing that Villa was doing its best to meet her needs and wishes. It meant the world to us to feel that our mom and grandma was treated like family in your home.

Sincerely,

I was distressed to learn my aunt needed a facility to help with her situation, and after asking for recommendations, Ramona received a high mark. I had contacted numerous facilities in the area and I must admit from the initial get-go at Ramona, the admission process, thoroughness of information, and assistance in responding to my inquiries were most assuring and professional. Just a few days later when we arrived, everything was all at the ready! I was most impressed with the cheery presentation of the unit, a departure of what I observed at the other skilled nursing facilities I had visited. Ramona seemed much cleaner and fresher. The thoroughness of the nursing staff in reviewing and questioning the transferred medical records gave me an even greater feeling of confidence in my choice. The responsiveness of physicians, attentiveness to get my aunt into the activity and community rooms to brighten her day, and her reported ease of being has relieved the weight of such an unpleasant task on our family and has assured us that she is in good care. I am quite pleased.

My husband, Robert has now been at Petaluma Care & Rehab. for almost 8 months. During that time his care has been excellent, his health has improved and our evaluation of the facility would be that it is very good. Bob appears to have no complaints about the food, his personal care or attention to his individual health needs. He has received therapy for getting "back on his feet". I think the activities department. does an excellent job of providing entertainment for the patients... We enjoy being able to so easily go outside to capitalize on the fresh air, sunshine and beautiful view.

My assessment of Petaluma Care & Rehab. would be that it is a very fine facility with caring and knowledgeable staff members, concern for comfort and cleanliness and ultimate interest in the well being of each patient."
